Prior to consultation appointment, complete this form.

Is your MTM apparel needed for a specific date? If so, please include that date. MTM clothing takes time, so we ask that you begin the process at least 3–6 months before you need your garments

Describe what kind of apparel you are looking to have made to measure: dress, skirt, blouse, etc.

Describe in detail the style, fabric choice (linen, cotton, etc), color, and if you have a pattern link or website I can refer to, please include it in your answer.

When you generally purchase a clothing item, check which size you would normally buy.
